This Privacy Policy explains how Miller Family Medicine Group ("we," "us," or "our") handles information collected through our website at millerfamilymedicine.com (the "Website"). It does not cover how we handle your protected health information as a patient of the practice — that is addressed in our HIPAA Notice of Privacy Practices, which is provided at the office and available on request.

This policy applies to information collected through our Website. It does not apply to:

  • Health information you provide during in-person visits, phone calls, faxes, or the Patient Portal — see our HIPAA Notice of Privacy Practices.
  • Information collected by third-party services we link to, such as the Patient Portal (healow), our Facebook page, or Google Maps. Those services have their own privacy policies.

2. HIPAA and your medical records

Miller Family Medicine Group is a HIPAA-covered health care provider. Your protected health information ("PHI") is handled in accordance with the Health Insurance Portability and Accountability Act (HIPAA), Pennsylvania law, and our Notice of Privacy Practices, which is given to patients at the time of first visit and available anytime on request. That notice — not this website policy — is the authoritative document describing your rights over your medical records.

This Website itself does not collect PHI. Please do not email PHI to us. Use the Patient Portal or call the office for anything involving your health information.

3. Information we collect

Information you provide

If you email us at officecontact@millerfamilymedicine.com, we receive your email address and any content you include. Please do not send protected health information, Social Security numbers, or financial information by email, as ordinary email is not a secure channel.

Information collected automatically

When you visit the Website, our hosting provider may automatically log standard server information such as your IP address, browser type, device information, pages viewed, and referring URL. This information is used for security, troubleshooting, and understanding site usage. We do not use this information to identify you personally.

  • Patient Portal (healow / eClinicalWorks): Access to your patient records is provided through healow. Its use of your information is governed by its own privacy policy and by any authorization forms you complete at our practice.
  • Facebook: Our Facebook page is subject to Facebook's own privacy policy and terms.
  • Google Maps: Directions to our office are provided through Google Maps, subject to Google's privacy policy.

7. Cookies and tracking

Our Website currently uses only technically necessary cookies required for basic functionality. We do not use advertising cookies, cross-site trackers, or behavioral advertising on this Website. Your browser allows you to control cookie behavior through its settings. If we add analytics or other tracking in the future, we will update this policy.

8. Children's privacy

Our Website is not directed to children under 13, and we do not knowingly collect personal information from children under 13 through the Website. Information about pediatric patients is collected through the office intake process, subject to our Notice of Privacy Practices and applicable law. If you believe a child has provided personal information through the Website, please contact us so we can delete it.

9. Data security

We use commercially reasonable measures to protect information collected through the Website. However, no method of internet transmission or electronic storage is completely secure. Do not send protected health information, Social Security numbers, or financial information by unencrypted email. For secure communication about your care, use the Patient Portal or call the office.

  • Medical records: For requests about your patient records — copies, amendments, restrictions, or accounting of disclosures — please see our HIPAA Notice of Privacy Practices or contact the office directly.

11. Pennsylvania residents

Pennsylvania does not currently have a comprehensive consumer privacy law comparable to California's CCPA. We comply with applicable Pennsylvania laws, including the Pennsylvania Breach of Personal Information Notification Act. In the event of a breach affecting your personal information, we will notify you as required by law.

Pennsylvania law provides additional protections for certain categories of health information (including HIV, mental health, and drug and alcohol treatment records). These protections are described in our HIPAA Notice of Privacy Practices.

We may update this policy from time to time. When we do, we will update the "Last updated" date at the top of this page. Changes take effect when posted. Your continued use of the Website after changes means you accept the updated policy.
